Fourth-degree burns include similar severe damage to the dermis, the underlying layer of skin. The damage in a fourth-degree burn can extend into the muscle, fat, tendons and the bone. This type of burn produces charred skin that requires grafting and results in severe scarring. Fourth-degree burns are so severe that they can result in amputation or worse, death.
